Our Case Management and Housing Navigation Team is prepared to serve you

Intensive Case Management Services (ICMS) provide comprehensive, individualized support to individuals experiencing homelessness or housing instability who require a higher level of care coordination. Through a client-centered and housing-focused approach, our ICMS team helps participants secure and maintain stable housing while addressing complex barriers such as mental health challenges, substance use disorders, chronic health conditions, limited income, and documentation issues. Case managers work closely with each client to develop personalized service plans, coordinate medical and behavioral health care, connect to public benefits, support employment readiness, and build essential life skills. The goal of ICMS is not only to stabilize housing but also to promote long-term self-sufficiency, dignity, and overall well-being through consistent, relationship-based support.
